Remember to compile the kernel with?

options         IPFIREWALL
options         IPDIVERT

gateway_enable=YES in rc.conf?

For testing, you may also want to make sure your firewall is
completely open, especially since I believe the default ipfw script
(if that's what you're using), even if set to 'open', blocks incoming
traffic from 192.168.x.x on the outside interface.
