I'm not sure if this helps, and it is exactly not what you asked for
(experienced advice?), however, this will get the job done:

Create a PDF file using the sgmlfmt(1) from the FreeBSD sgmlformat package: 

$ sgmlfmt -f ps file.sgml
$ ps2pdf file.ps
