We used qmail + vpopmail + MySQL setup in the past, but switched to Exim + 
MySQL setup about a year ago.
Current setup consists of classic Exim + Courier (pop[s]/imap[s]) + MySQL + 
ClamAV + SpamAssassin combo. Exim and Courier-Imap both use MySQL.

Exim integrates very well and easily with SpamAssassin and ClamAV for 
filtering purposes.
