FYI: Doc builds now generate index

Hello

Just FYI, the doc builds for the FTP site and for the web site now
generate document index for the documents which has one (ie. GEN_INDEX
is now set).  This should make the indexterm's, mainly in the
Handbook, somewhat more useful.
