Missing a step in new(er) Remote gdb/kdb setup?

In my case those are caused by protocol time-outs. This typically  
when I start kgdb before I switch the kernel over to the GDB backend.  
a GDB problem. I just retry and everything is ok.

Don't know. I never use sio(4). I always use uart(4).

You can't. It's a bug. The bug is mostly that libthread_db.so is loaded
when it shouldn't. I didn't write that code.
