Yamaha CRW6416 not recognized

I have a Red Hat Linux 6.2 system, running the stock 2.2.16-3 kernel from
Red Hat's update site. This system has an Adaptec 2940 Ultra Wide adapter
with 3 internal scsi disk drives and 3 external devices in a scsi enclosure
cabinet: a 4mm DDS3 tape drive, a TR4 tape drive, and a Yamaha CRW6416S
CD-RW drive (id 4).
When the system boots, the Adaptec BIOS detects all 6 devices. However,
the Linux aic7xxx driver does not see the Yamaha CRW6416S CD-RW drive.
I've searched through the list archives and the web for reports of this
type of problem. The closest thing I found was the "probe all LUNs"
problem in the kernel that shipped with Red Hat 6.0. The fix there was
to update to a kernel wherein the "PROBE ALL LUNS" option is disabled.
For example, the kernel that shipped with Red Hat 6.1.
Below, I've appended the output from /proc/scsi/scsi and
/proc/scsi/aic7xxx/0.
Does anyone have any suggestions? TIA.
