The most obvious place I’ve seen incorrect results has been when multiple
different locks use the same name. Is it possible that is a factor here?

(Note that this can be somewhat pernicious. For example, if two different
locks use the name “foo” and then one of those is held while locking “bar”,
then all of the locks ever held while locking either “foo” lock will form
part of the locks witness thinks are OK to hold while locking “bar”.)
