Hi Ruben,

Can you confirm that "cpl does not map to outstanding cmd" was the first
unusual line to show up?  (i.e., there weren't more lines before that which got
lost?)  We had a hypothetical explanation for this which should have also
involved having a "missing interrupt" message at the same time...
