Are these the only kernel options I need to enable altq on FreeBSD 5.2.1

ALTQ support was added June 13th 2004. It will be part of the 5.3 Release.

Yes, depending on your needs of course. For SMP builds you will also need 
"options ALTQ_NOPCC".
