I'm unable to reproduce this problem locally even with your .sawfishrc
snippet.  The middle mouse menu pops up quickly every time.  I even tried
launching a few apps from it.

Can you try removing rep-gtk if you haven't done so already, then reinstall
rep-gtk2.  Also, make sure you don't have NOLIBC_R defined in /etc/make.conf.
You may also try without a custom .sawfishrc, and see if that helps things.
