help needed 2940U2W, aic7xxx 5.1.15, kernel 2.2.5 scsi

Couple of things.. I don't know if it would effect you but the latest drivers are 5.1.18....

Also, have you tried mke2fs without bad block checking?  Not that this is a solution but it may give you an idea where the problem lies..
