Changelist template changes

I've recently added the following lines to the changelist on repoman.
The Description block should look like this:

Description:
        <enter description here>
        PR:
        Submitted by:
        Reviewed by:
        Approved by:
        Obtained from:
        MFP4 after:

Just like the CVS version, you should feel free to leave them blank,
they will be stripped off if they don't have anything added to them.

Finally, the MFP4 doesn't have a script backing it like the CVS
version does, if that sounds interesting, I might see what it would
take to implement.
