goto http://www.freebsd.org/send-pr.html, fill out the form, click on "Submit Problem Report".

The browser will send the problem report, but it will never show a page telling the user about successfully having send the problem report. This page seems to load, but there is only a few byte send out by the server.
In tune the user will think sending failed. If he/she clicks the button again, he/she is presented with a page telling about an incorrectly entered captcha. The user will try to correct the captcha, but with the same result (since the problem report was send and the captcha declared "used".

The user might now stop, or if he/she is willing trying to send the record again, using a new page, producing duplicates, since the problem report was send successfully, just no page given back from the server telling the user about.

I could see this behavour with
- Opera 9.10
- Seamonkey 1.1.1
