Trouble with php and submit button

Hi there, my php code works fine on any server except on mine :(

Think there's a problem with the installation of php, whenever I push a button, nothing happens... At first I thought it was the php code, but the code works fine on other servers. Maybe som1 has any id of what I've done wrong during install?

This is what i'm running:

Freebsd 4.8
mod_php4-4.3.3,1    
apache-1.3.28
