I'm now seeing these in stable/11, as of r343948.  I was running r343926
yesterday, which had some issues with iwn(4), but I don't know if this issue
was involved.  (The laptop was attempting some backups while I was elsewhere.) 
I am fairly sure I did not see it often in stable/11 at r343901.

stable/12 and head exhibit the issue so frequently that this is the main reason
I haven't migrated my systems from stable/11 to stable/12.
