I came to know about this issue from this mail thread [1]. I went through all
the comments mentioned here and given the comments form both tech-lists and
Gerard Seibert, starting from here [2], I did this analysis:
Mike Karels => Gigabyte MB => GA-X99P-SLI (rev. 1.0)
Gerard Seibert => Dell XPS 8930 => 0T2HR0
tech-lists => Supermincro MB => X9DRE-TF+/X9DR7-TF+
Possible Root Causes:

the common details amongst all reported cases so far.
Trying to test the 1st possible root cause (Having a mix of USB 2.0, USB 3.0
and USB 3.x ?), I ran tests on:
Lenovo Thinkpad W540:

Chipset: Mobile Intel QM87 Express (having USB 2.0 and USB 3.0)
I tried different USB UEFI Setup Configurations, but I didn't see any of the
errors mentioned.
That lead me to the path that it could be a mix of both Possible Root Causes,
hence I did a bit of research and I've found these 2 machines available for
purchase:
Dell XPS 8920:

I guess that the Dell XPS 8920 one looks like a better test case, but your
input would be highly appreciated.
Also, I want to take the chance and learn more as I want to contribute to the
FreeBSD project, so it would be great if you can provide me with guidance and
mentorship.
