Warner, it is unlikely that it had to do with locking, but not impossible, of
course.  If you look at attachment 191533 you can see that INQUIRY ccb was
failing for some unknown reason, but that happened only when CAM debugging (via
camcontrol debug ... all) was enabled. Quite strange.
