If the only reports were about the 'kpf 2' during the failure, please try the
new debugging patch.  It still requires setting of debug.vn_io_fault to zero.
As before, I need the dmesg output of the failing episode.

Patch is against HEAD, and it would be extra good if you tried this on the HEAD
kernel, no need to update usermode.
