Put something like:

HTTP_PROXY=http://proxy.example.com:3128

...into your shell environment or /etc/make.conf, depending.  You can  
also set fetch's environment specificly, and/or nest them, like:

#FETCH_ENV=     FTP_PROXY=http://proxy:8080
