I'm not quiet sure, but I think you'll find Asterisk and SER interesting.
Check:
Asterisk: http://www.asteriskpbx.com
SER: http://www.iptel.org/ser/

They're both available in the ports collection:
ports/net/asterisk and ports/net/ser.

VOIP info http://www.voip-info.org can be very helpful as well.
