There are a lot of places to begin by reading.  The most important is 
/usr/src/UPDATING. For version 4.x, the sequence begins around line 482. 
Look for

       To update from 4.0-RELEASE or later to the most current
        4.x-STABLE

There are a number of books that tell you what to do but UPDATING is the 
final reference. If it doesn't make sense to you, you need to read the 
handbook sections on updating your world.
