Hello Nathan,
After I 've tried with that version, and luckily it worked (ls command works properly, finally!)
So it's almost definitely true that there's a confusion of mkisofs instead of makefs you have talked about in the version I have first used.
Actually, that buggy version must be better removed from the download list completely.

The livefs images generated with the old release generation code were
broken due to some bugs in mkisofs generating hybrid images. You'll need
one of the newer ones instead (which use makefs instead of mkisofs):
