why was /etc/defaults/make.conf not updated

Could be many reasons, e.g.:

You didn't do a mergemaster while upgrading the base system,

or you upgrade from 4.x to 5.x, which no longer has /etc/defaults/make.conf

...

But either case, looking at /usr/src/UPDATING and /usr/ports/UPDATING
is a good idea.
