Neither databases/ip4r or postgresql93-contrib-9.3.9_1 install ip4r.control
which is required afaik.

Manually creating /usr/local/share/postgresql/extension/ip4r.control with
contents:

# ip4r
default_version = '2.0'
relocatable = 'true'
module_pathname = '$libdir/ip4r'

resolved my issue.
