My work is almost same. But I think to try to use
displayNamedCIDFontTT like following this line.

displayNamedCIDFontTT Ryumin-Light-H "/usr/X11R6/lib/X11/fonts/TrueType/kochi-mincho-subst.ttf"
displayNamedCIDFontTT GothicBBB-Medium-H "/usr/X11R6/lib/X11/fonts/TrueType/kochi-gothic-subst.ttf"

But xpdf-3.00 was broken handing TrueType fonts. So xpdf cannot
display Japanese PDF. Now pending...
# I know how to fix, and patche(s) are located on Debian... but...
