rl(4) style, locking patches

I have several patches here which appear to work OK on UP with
debug.mpsafenet enabled (and should be applied in this order):

These clean up the style of rl(4), and eliminate the use of MTX_RECURSE as
well as setting the INTR_MPSAFE flag. Many thanks to wpaul@ for doing
the bus_dma(9) cleanup.

Feedback and review would be much appreciated...
