snd_hda, sound in both speaker and headphones

I am sorry if it's just me being stupid, but how can I do this when 
/usr/sbin/mixer does not recognise "speaker"?

Mixer mic      is currently set to   0:0
Mixer cd       is currently set to  52:52
Mixer rec      is currently set to   0:0
