I had a number of issues with Qmail.  Most were fixed through the 
judicious application of various patches, but in the end, keeping up 
with all the patches proved to be too much work.

The final solution I came up with, was to switch to Exim, which, as an 
MTA, is much more complete, robust, and extensible.
