After upgrading to a new motherboard I ran into the same situation
(new on-board card and SB Live!) and created a patch for
gnome-volume-control that shows real device names instead of Card 0,
Card 1, ...

I've send the attached patch to the maintainers (the uni-bielefeld.de
address bounces and iain at ximian.com didn't respond).
Perhaps Joe might add this to the FreeBSD tree.
