After confirming bug 211684 fix try wiping ~/.config/pulse then restart the
server: "pacmd exit" then reload browser tab. Otherwise, not enough details.

2016Q3 package set as part of 11.0-RELEASE media is NOT supported anymore. Make
sure you're using 2016Q4 or /head.
