the previous issues (xpt_action_default and DIOCSKERNELDUMP) have been fixed.
however this one, where a lot of apps fail with errno -128 when accessing the
disk, still exists. this really needs to be fixed before 9.0!

...again...reverting to r222890 solves this completely. so it's not a problem
with the disk or anything.

..i'm guessing this is ahci related, but i'm not sure.
