ulpt problem (USB_ERR_IOERROR)

This looks a bit like an issue I'm about to start chasing where FS 
devices fed through a HS hub disconnect "spontaneously" under load.  It 
doesn't look like it but to be sure there are no hubs between the host 
and the printer?
