This shows the 'vgapci' driver has attached to your card.
Is the nvidia.ko loaded before the kernel starts? (ie do you have
nvidia_load="YES" in /boot/loader.conf?)
It would appear the latest driver should support your card (from the
README) and the PCI ID matches what is there.

You should rebuild the nvidia port every time you update your kernel,
and you should keep your kernel & userland in sync if at all possible.

Was it the same version of the port?
