pccard modem don't work

Please post an enitre dmesg booted -v with the tunables
'hw.pccard.debug=1' and 'hw.pccard.cis_debug=1'

It appears that sio didn't like this card for some reason, and
bootverbose and/or the pccard debugs will help track down why.
