I'm having this problem on 12.1-RELEASE-p2 GENERIC, with a Supermicro X11DPi-N,
6 drive 3-mirror stripe zpool of "WD Gold" disks.

There's significant load and I haven't had disconnects yet, but it's only been
up for 8 days and I've rebooted a few times to test the loader.conf tweaks
(which didn't help as far as I can tell).

I'll try replacing the controller next week, since I haven't heard of this
issue without roughly this drive/controller combo—if that's a bad idea please
let me know.

The errors pop up on seemingly random disks, e.g.
