We are seeing the same problem across a large number of machines, using
Supermicro X11SPW-TF and X10SRW-F motherboards.

The disks are:
WD1004FBYZ-01YCBB1
or
WD1005FBYZ-01YCBB2

And they just fall off the bus and won't come back (not even visible in the
BIOS) without a hard power cycle.
