I have exactly the same problem on near the same hardware:

Dell PowerEdge R710
128GB Memory
H200 HBA 
6 x 2Tb Drives.

The problem appear right after upgrade to 11.1-RELEASE
The last drive is not visible by OS, but looks completely OK in BIOS.
