FreeBSD 12 (releng) kernel built with the following options, as documented in
https://www.freebsd.org/doc/en_US.ISO8859-1/books/handbook/firewalls-pf.html
section "30.3.2. Enabling ALTQ":
ALTQ ALTQ_CBQ ALTQ_RED ALTQ_RIO ALTQ_HFSC ALTQ_NOPCC ALTQ_PRIQ (extra option)

This results in a failure to load the module:
kernel: link_elf_obj: symbol altq_add undefined
kernel: linker_load_file: /boot/kernel/pf.ko - unsupported file type
