These are in the header files.

I wonder if they should be defined as 1?  Because the linker is complaining
about a missing symbol, it suggests the accessors are functions and not macros.

What's the side effect if we make the library NCURSES_OPAQUE?  It sounds like
it doesn't change the ABI but will potentially break certain applications that
accesses the internals directly?
