The machine is a Dell Inspiron 530 with Core 2 Quad CPU (Q6600), 3GB RAM.  This is definitely an inexpensive consumer-grade system with Phoenix BIOS (Dell version 1.0.15, the latest).  I seem to think it may be made by Foxconn.

When booting GENERIC from hard disk, it almost always prints

pauses a few seconds, then prints

  AP #1 (PHY #1) failed!
  panic y/n? [y] panic: bye-bye
  cpuid = 0

Once in every 5-10 boots, it starts successfully.  

When booted from CD-ROM, it always starts successfully.  I can also boot disc1, break from the boot menu to a loader prompt, 
  unload
  set currdev=disk0s1a:
  boot
and get a successful boot that way.

Setting kern.smp.disabled causes it to work, too, of course...

The machine is a USB-only system, no PS/2 ports, so I attribute the oddness at the prompt to having no keyboard available.

Sometimes it rapidly scrolls text that might be a register dump on a trap and reboots in less than half a second, but I haven't seen that in a while.  I think that's post-panic behavior.

I've started looking at boot0/boot1/cdboot to see if I can see anything obvious, but nothing's obviously wrong.  I'm also willing to help another developer with testing/debugging this one.

This appears to be same/similar behavior as a problem that was seen a couple of years ago with Intel SE7500CW2 motherboards (we have a system here with an SE7501CW2 motherboard that we occasionally see this on.)
