We first found this in RELENG_4, with this motherboard, drive and CD, we have confirmed it on multiple systems. With the HD as Master and CD as slave (via cable select or jumperd) we get "ata0-slave: ATA identify retries exceeded"
(boot -v from 4.11 included bellow)
However if we make the CD master and HD slave via cable select or jumpers all works fine.
In RELENG_6 a regular boot on the vga console panics while trying to identify the ata0 slave when CD is slave. However boot -v on the vga console works as does any boot on a comconsole. (Does this suggest some kind of timing problem?)
Again making the CD master and HD fixes this.
In RELENG_4 we found either using a rev 2.6 MB, or replacing either the HD or CD with another model (IBM deskstar HD or an old HP CD-R) would also work round the problem.
Linux and windows work OK with CD as slave or master.
As the panic doesn't occur when using the serial console I only have it as a JPEG which I can email on request.
Hyperterminal rather mangled the RELENG_6 boot -v output but this is the ata device section:
