While playing around with send-pr (yes, my fault), I ran into this:

        s)end, e)dit or a)bort? s
        sed: 11: "
        /^>Organi ...": unterminated substitute pattern
sendmail: fatal: No recipient addresses found in message header
        send-pr: mysterious mail failure.
send-pr: the problem report remains in /tmp/pbad.5UUMoa9S and is not sen

        [~] edwin at k7>file /tmp/pbad.5UUMoa9S
        /tmp/pbad.5UUMoa9S: empty

So no saved problem report!
