Format harddisk with AIC-7850

My linux box has the following things.
--------------------------------------

- on board SCSI controller AIC-7850
- Micropolis SCSI 1624 (668 MB, Bytes per Sector 512, Cylinders 2,112)
- SCSISelect BIOS v1.11.02

My initial Linux disks (Slackware 3.4).
--------------------------------------

- bootdisk : aha2x4x.s
- rootdisk : color.gz


I add the boot option "ramdisk aic7xxx=extended"
------------------------------------------------
The following message come out.

- aic7xxx: Reading SEEPROM... Unable to read SEEPROM
- aic7xxx: extended translation enabled

I can fdisk and mkswap but I can't format the harddisk

When I format the drive, the following message come out
