Some basic questions first:

Is the device connected to a USB root HUB or an external USB HUB?

Do other devices connected at the same USB speed exhibit the same problem?

If the HUB reports a disconnect event, there is very little we can do about it.

Try to also enable host controller debugging, XHCI/EHCI/OHCI/UHCI .
