I'm closing this as rhurlin identified the root cause: an incomplete
tex-kpathsea installation was missing texmf.cnf. The "File exists" error during
reinstall was a side effect. The reporter resolved it by updating related TeX
ports. This was a dependency ordering issue, not a bug in texlive-tlmgr.
