patch for addin usecallmanager to the config menu of net/asterisk20 and
net/asterisk22

Attached patch adds the popular/widely used usecallmanager
(https://usecallmanager.nz/documentation-overview.html) patches (common in
Cisco estates) to asterisk.

Has been used/tested in production for over a year now (on asterisk20) and
followed all version updates perfectly.
