Updated from thumbnail_index with fixes/adds.

Bugs/additions
1. Subdirectory names ending in .jpg etc mishandled.
2. Compressed files (ending in .Z) mishandled.
3. Two files with same basename mishandled (eg, xx.jpg and xx.gif or xx.JPG).
4. Special files (eg, sockets, fifos) mishandled.
5. Added support for .HEIC & .MOV files from iPhone.
6. Added "Back-to-top" links.

Tested on 13.1-RELEASE-p7. Updated file with these fixes & adds attached.
