In the commit log the keyword is "Changelog" not "Changes". 

Check the file https://cgit.freebsd.org/ports/tree/.hooks/prepare-commit-msg
for references or use it to create the commit logs.

You can do this to use the hook:
git config --add core.hooksPath .hooks
