I interpreted "copied over" as copied from the caller instead of the target. This is probably because until now I always assumed that keepenv was only meant to pass environment variables from the caller to the target (and have no real effect on doas -S). In my opinion, this alternative meaning of keepenv (when using -S) remains a bit inconsistent. But the change in the manual page at least makes this explicit; it now makes clear what to expect when using -S. So as far as I'm considered this resolves the issue.
