It is timing-related, so is is more or less likely to happen depending on the
combination of hardware in the system, version / optimization level of the
compiler being used, number of CPU cores / threads, etc. As I mentioned when I
opened the PR, Dell Service moved the card from one slot to another and the
problem (temporarily) went away.

The "Out of chain frames" message is likely a red herring - something is
happening on the card or system that causes card initialization to think we
have no chain frames, possibly because the card hasn't finished initializing
yet.
